Introducing Wellness my Way
A program focused on giving the Maranoa community a healthier future
Wellness my Way is a free, personalised approach to improving health and wellbeing across the Maranoa community including Roma, Surat, Mitchell, Injune and Wallumbilla.
A single ‘front door’ to prevention programs, Wellness my Way connects the community with the health programs most suited to them.
Simply take the free online health check. Then a telephone-based coach will contact you to develop an action plan that connects you to free or low-cost health and wellbeing programs suited to your needs.
If you would prefer to complete the health check over the phone, call 13HEALTH (13 43 25 84) and ask for the Way to Wellness service (Monday – Friday, 8am – 8pm).

How does the program work?
By completing the free online health check, you can check in on your current health and wellbeing and receive personalised support through a telephone coach.
The coach supports individuals to develop a tailored health action plan, and to continue their health journey through a recommended program or resources that can be accessed online, via phone or face to face in the community.
Individuals are encouraged to complete the health check online via their phone, tablet device or computer. A telephone-based option is also available by phoning 13HEALTH (13 43 25 84) and asking for the Way to Wellness service (Monday – Friday, 8am – 8pm).
Who are we?
Wellness my Way is a joint initiative of Health and Wellbeing Queensland, South West Hospital and Health Service, Queensland Health’s Health Contact Centre and The Health and Wellbeing Centre for Research Innovation at The University of Queensland. Together, we are co-designing, implementing and evaluating Wellness my Way as a pilot project that delivers an adapted version of the Health Contact Centre’s Way to Wellness health assessment and phone coaching service.
South West Hospital and Health Service will lead stakeholder engagement and implementation within the Maranoa region to ensure local relevance and sustainability, and community voices will inform implementation through local consultation and a continuous quality improvement approach.

Wellness my Way is an initiative of the Queensland Government, delivered in partnership by Health and Wellbeing Queensland, the Health Contact Centre and South West Hospital and Health Service.
