Take charge of your health with Wellness my Way
Eat well, move more, quit smoking, reduce alcohol and boost wellbeing today.
What is Wellness my Way?
Available to South West Queensland, Wellness my Way is a FREE entry point to health and wellbeing support in your community.
By completing a health check, Wellness my Way connects you with a telephone coach who helps you make a personalised plan and link with local programs, services and resources that fit your goals.
Whether you want to eat well, move more, quit smoking, or improve your overall wellbeing, Wellness my Way helps you find what’s available in your community – and gives you the support so you don’t have to do it alone.
The health check can be completed online at any time. If you would prefer to complete the health check over the phone, call 13HEALTH (13 43 25 84) and ask for the Way to Wellness service (Monday – Friday, 8am – 8pm).

Who are we?
Wellness my Way is a joint initiative of Health and Wellbeing Queensland, South West Hospital and Health Service, Queensland Health’s Health Contact Centre and The Health and Wellbeing Centre for Research Innovation at The University of Queensland. Together, we are co-designing, implementing and evaluating Wellness my Way as a pilot project that delivers an adapted version of the Health Contact Centre’s Way to Wellness health assessment and phone coaching service.
South West Hospital and Health Service will lead stakeholder engagement and implementation within the region to ensure local relevance and sustainability, and community voices will inform implementation through local consultation and a continuous quality improvement approach.
