An innovative, scalable approach to chronic disease prevention and management

Logan Healthy Living is an innovative, community approach to chronic disease prevention and management, with a focus on supporting those at risk of, or living with, type 2 diabetes. Delivered by UQ Health Care in partnership with Health and Wellbeing Queensland, and supported by alliance partners, the service is an exemplar of effective cross-sector partnership and provides a blueprint for integrated chronic disease prevention and care with proven outcomes.  By working together, we can expand access to preventive healthcare, empowering individuals to proactively manage their wellbeing.   

Almost 54% of Queenslanders had one or more chronic conditions in 2022 posing a significant challenge to the health system. In Logan, type 2 diabetes is the most common cause of potentially preventable hospitalisations, occurring at a rate 40% higher than the national average. To respond to this urgent need, the Logan Healthy Living Lifestyle Management Program was co-designed with Logan residents and stakeholders, ensuring a targeted, place-based approach which addresses local community need. 

Logan Healthy Living service offerings

Logan Healthy Living is transforming health care within Logan. The service integrates care across specialist services, general practitioners and community services to provide a coordinated approach. Led by an interprofessional and student-infused allied health workforce, the program promotes holistic, person-centred care, which fosters continuity through collaboration across disciplines.  

Involving future healthcare professionals through integrated student placements from multiple tertiary education institutions not only strengthens the local health workforce but also contributes to building a more preventive, sustainable healthcare system for Queensland and beyond.

Rising Risk of Diabetes Program

For community members at risk of developing type 2 diabetes, heart disease and stroke, Logan Healthy Living  facilitates, My Health for Life, a free health coaching program designed to help individuals discover ways of changing day-to-day behaviour.

Recently Diagnosed Program

For community members diagnosed with type 2 diabetes within the last 6 years. This person-centred approach helps people manage their diabetes early to reduce risk of complications and, if possible, achieve diabetes remission.

Complex Type 2 Diabetes Program

This program supports community members living with type 2 diabetes, including with complex comorbidities or complications. The program responds to the unique needs of complex type 2 diabetes to support self-management.

Youth Program

The Youth program is open to youth aged 16-19 living with type 2 diabetes. The program provides a continuum of care for adolescents transitioning between child to adult services. 

Service outcomes

Health and Wellbeing Queensland supported Logan Healthy Living through a 4-year pilot from January 2021 to December 2024. The pilot was embedded with a comprehensive research and evaluation framework, demonstrating positive results across: 

  • program reach 
  • program effectiveness 
  • economic impact; and 
  • health system impact.

So I’ve grabbed it with two hands, because without it, I wouldn’t do it. It’s great for your health. We’re not pushed into things, everything is suggested with your best interests in mind.
