
ConnectingQ ECHO® Session – Learnings from Wellness My Way pilot in Maranoa region

Join us to learn more about Wellness my Way – a program focused on giving the South West Queensland community a healthier future.
Wellness my Way is a joint initiative of Health and Wellbeing Queensland, South West Hospital and Health Service, Queensland Health’s Health Contact Centre and The Health and Wellbeing Centre for Research Innovation at The University of Queensland. Together, we are co-designing, implementing and evaluating Wellness my Way, a pilot project that delivers a consumer-led, single ‘front door’ to chronic disease prevention, connecting community members with health information and prevention programs that meet their individual health needs and priorities.
During this session you will learn about the benefits of the collaborative partnership, community consultation methods to ensure local relevance, and the continuous quality improvement approach to implementation. You will also learn what early data and insights are telling us, what worked well and presented challenges, and how this work can be sustained into the future.
