Wellness my Way early findings report

View early insights from the Wellness My Way pilot by Health and Wellbeing Queensland.
Health and Wellbeing Queensland, in collaboration with South West Hospital and Health Service, Queensland Health’s Health Contact Centre and The Health and Wellbeing Centre for Research Innovation at The University of Queensland, has released early insights from the Wellness My Way pilot.
The program provides a single-entry point to preventive care, connecting people to telephone coaching, local services and statewide support programs.
What we found
Over 11 months, the South West community had strong engagement with the program:
- 383 health assessments completed (including 96% online)
- More than 1,250 personal health goals set
- 252 referrals made to free or low-cost nutrition, physical activity, mental wellbeing and behaviour change support programs
- 47% of participants progressed to telephone coaching.
The findings highlight the program is reaching people at different stages of their health journey. Three in 4 participants were at risk of Type 2 diabetes within 5 years and more than 1 in 4 were already living with a chronic condition. Wellness My Way is connecting people both before and after diagnosis, creating valuable opportunities for prevention and early intervention.
Stakeholder feedback has also shown that local “connectors” (community members who help the program reach more people) were key to raising awareness, building trust and driving engagement. The program has strengthened collaboration between health and community organisations, increasing awareness of local services and creating new referral pathways.
Next steps
The program will continue in South West Queensland for another 12 months. Priorities include expanding the connector network, refining engagement strategies and continuing local promotion. A comprehensive evaluation report will be released in mid-2026.
